Patient Care Coordinator Full Time

Position Goal: 

The Patient Care Coordinator works under the general direction of a physician utilizing communication and management skills to coordinate seamless care delivery for patients. Will act as a patient advocate to assure the patient's access to health care during their entry into and through the services of the clinic.

Position Responsibilities:

  • Act as the liaison for referring practices for ease in access and integration of patients into the surgeon’s clinic.
  • Act as contact for clinic providers, patient and family, outside agencies, outside staff and general public by providing information regarding the services provided by the physician.
  • Act as liaison between the patient and the various clinic areas, ancillary and Hospital departments to coordinate activities involved in pre-operative planning and preparation or planned hospitalization.
  • Coordinate pre-admission tests, pre-surgery appointments, and required ancillary procedures, and surgery as delegated by the surgeon.
  • Ensure a complete medical record is present at the time of patient visit, admission, and/or procedure including procurement of records as warranted from referring provider(s).
  • Facilitate the flow of documents between patient, providers and payers.
  • Provide prompt and accurate surgery scheduling, authorization for services through to timely billing.
  • Act as the clinic’s medical assistant during clinic hours performing the job functions consistent with the medical assistant including: obtaining patient medical history, vital signs, chart preparation, medication reconciliation, patient allergens, and assisting with medical examinations, performing routine specialized procedures, such as suture/staple removal and dressing changes and phlebotomy as needed.
  • Clean and stock exam rooms and change sheets on exam tables after each patient.
  • Establish contact with existing, newly diagnosed, potential, or referred patients by facilitating access to surgeon, educational materials and communication with care providers.
  • Follow up calls to patients after surgery to document their progress and optimize the patient experience.
  • Use critical thinking skills to troubleshoot patient issues or concerns.
  • Constant monitoring of schedules and patient flow during clinic to ensure maximum efficiency/organization.
  • Clean and sterilize equipment using approved techniques.
  • Perform administrative duties to include but not limited to: collection of past due balances and prepayments when scheduling surgery, scheduling appointments, procedures, and surgeries, pre-cert procedures, insurance verification, data entry, posting charges.
  • Complete insurance, disability, and FMLA forms in a timely manner.
  • Implement and maintain OSHA education and guidelines.
  • Actively pursue appropriate continuing education and certifications.

Experience: 

Required:

  • 2 years of relevant experience in a clinical setting

Preferred:

  • 1 year of surgical and Orthopaedic experience

Education: 

 

Required:

  • High School Diploma or equivalent

Preferred:

  • Education required for nursing licensure

Special Qualifications: 

 

Required:

  • BLS certification from American Heart Association or American Red Cross
  • Must be able to work independently and have working knowledge of HIPAA requirements.
  • Effective communication and interpersonal skills.
  • Excellent critical thinking skills.
  • Excellent customer service experience.
  • Knowledge of the health care field and medical specialty.
  • This position may require travel to and from satellite clinics as assigned. Please note, if assigned for travel, this position will require a valid driver’s license and satisfactory Motor Vehicle Record upon hire (MVR will be screened on an annual basis).

 

Preferred:

  • Current state nursing license (LPN or RN)
