Quality Patient Care Coordinator

Position Goal: 

To manage the Quality and Value-Based Care programs in which Hughston Clinic Orthopaedics participates. Focus will be toward ensuring primary total joint arthroplasty (TJA) patients are guided through the entire surgical and recovery experiences to facilitate improved transitions and outcomes.

 
Position Responsibilities:

  • Manages patient surgical case care for any/all episodes under an alternative payment arrangement program such as, TJR for BPCI and coordinates with physicians and HCO clinical staff.
  • Works with physician lead for Quality Patient Care Programs and assists with communication/meetings between physician lead, HCO physicians and staff, and external care partners.
  • Communicates and trains HCO physicians and staff on clinical procedures to include the protocols and processes required by HCO participating programs.
  • Builds and maintains relationships with care partners in order to achieve stated goals.
  • Builds and maintains relationships with program initiators, such as payers, employers, and facilities in order to achieve stated goals.
  • Monitors, analyzes, and communicates with HCO physicians and staff regarding internal program data and external program reports (as provided by program initiators).
  • Develops patient communication workflow processes using any new systems for this purpose.
  • Reviews all program reports, identifies trends and processes to affect desired program changes, and communicates with physicians and staff regarding reports.
  • Trains and orients new clinical staff on program’s protocol.
  • Performs pre- and post-surgical telephone calls to assess patient needs and compliance.
  • Conducts comprehensive pre-admission assessments, education and follow-up to assure needed services are in place.
  • Follows the patient while hospitalized; escalates long stay/high cost cases and actively engages in potential disposition changes to assure appropriate discharge and follow-up.
  • Utilizes current patient communication workflow processes using any new systems for this purpose.
  • Oversees nursing care essential to the assessment, promotion, maintenance, and restoration of patient’s health and wellness.
  • Maintains patient and staff confidentiality.
  • Participates in professional development activities and maintains professional affiliations to include attending conferences and workshops.
  • Responsible for implementation of OSHA regulations.

Experience:

Required:

  • 2 years of clinical experience in a hospital or medical office setting or telephonic case management experience

Preferred:

  • Experience with discharge planning and outpatient care coordination
  • Experience with geriatric patients and skilled nursing home facility

Education:

Required:

  • High School Diploma
  • BLS Certification

Special Qualifications:

Required:

  • Must have a valid driver’s license and satisfactory Motor Vehicle Record (MVR).
  • Travel is required.

 Preferred:

State nursing or athletic training license
